Ensure commits are not lost

Currently, the emulated commits are always rewriting the same `CHANGELOG.md` file with the commit name.

We don't want to lose commits.

### What

We should ensure what's added to this CHANGELOG file is unique.

### How

Proposal: Add the original commit hash to the `CHANGELOG.md`
