Auto-Claude crashes on task completion (QA_PASSED → human_review transition)

What happened?

• AC v2.7.6 on Ubuntu 24.04 (XFCE/XRDP) crashes consistently when a task reaches the QA/completion phase
• Reproducible across multiple tasks
• Might this be somehow related to GPU subprocess crash on Linux Wayland (Intel Arc, Electron 40/Chromium 144) #1906? Did not find other related issues.

Investigated with CC:

• Last log lines show TASK_STATUS_CHANGE → human_review → crashpad elf_dynamic_array_reader error, then the process
  dies
• No OOM — system has 12GB+ free when it happens
• Running with --disable-gpu --disable-gpu-compositing --no-sandbox flags
• Electron/Chromium segfault appears to be triggered by the state machine transition updating a destroyed UI
  component

Steps to reproduce

Reproduces every time in my current setup, when a task moves to human review (from AI review). After restarting the app, the task looks ok and I can continue.

Expected behavior

The task should move from AI review to human review without crashing the application.
