Publish codex-flow to npm

Current install works through GitHub: npm install -g github:Dmatut7/codex-flow. Next step is publishing the public codex-flow package to npm after npm account login is available.

npm login succeeded as tutudma and publish preflight passed (typecheck + 19 tests). Actual npm publish is currently blocked by npm publish-time 2FA OTP: run npm publish --access public --otp= when the fresh email OTP is available.
