[Bug]: OpenCode can’t be switched out of plan mode

Steps to reproduce

1. Configure OpenCode to start in plan mode by default
2. Select an OpenCode model in dpcode
3. Ask to make a change, receive a plan, but the model asks the user to exit plan mode

I tried manually doing /plan and /default but it doesn’t seem like these do anything for OpenCode?

Expected behavior

Expected to be shown that the agent is in plan mode and a way to switch out of plan mode.

Actual behavior

Agent is stuck in plan mode and asks to be changed out of plan mode but I can’t seem to do that.
