Readme doesn't clarify button location

The root [readme](https://github.com/ForbesLindesay/github-real-names/blob/master/README.md) references a button to toggle between real names and usernames. However, it does not specify where this button is located.

I recommend adding a few steps to show where this button is located and then adjust the images to be before and after.
