support files without default namespace, again

#267 is back again!

After the refactoring XLSX can no longer read certain files that prefix their tags with "x:"

I've spent some effort to work around that issue, but it involves more bits than the last fix.
Moreover, there may be difficulties ahead for writing ...

Can you add any more clarification? I don't have a source of a file with no default namespace, I don't think, and there must not have been one in the test suite from when #267 was fixed or I'd have noticed when it broke again.

Edit: Also, I wonder if it works with the code in PR #361, which uses XML.jl v0.4. Although that code (a WIP) is twice as slow as the current release, it includes significant changes in the handling of xml files, including proper treatment of XPATHs.

I consulted my new best friend (Claude) for advice and was able to put something together which may work. I've nothing to test this with but offer PR #369 as a work in progress to see if it helps. Feedback would be much appreciated - or, better, a test file!

PR #381 now fixes this but for read only. It would be good to get some feedback. I will be working on write next.

Cool, I'll test this next week. My file is an automatically generated dataset, so I cannot easily share that. But I'll ttest and try to distill a testicle from it.

I've done some more work on this and think it now works for both read and write. The fix for write threaded through the code quite extensively which was a bit of a pain. However, it did reveal a few assumptions baked in to the code that are always true for files produced by Excel but may not be for files sourced elsewhere - like the two test files I now have. I've fixed all these now, too, so overall the code should be more robust to rare edge cases.

The PR with the fix is #382. I'll close #381.

I'd like to make a new release with this fix but I'd prefer to wait until you've had the chance to give it a quick test, too, @hhaensel, if you don't mind. Thanks!
