This likely needs wider support and UI support for picking the various variations too when manually equipping too, but for now we should probably just select the correct ItemModifiedAppearance.db2 row based on the ItemID and the item_appearance_modifier_id (ItemAppearanceModifierID column in ItemModifiedAppearance.db2) that is returned in the API to fix this issue in the next update with armor that has multiple variants (there are a lot of these).
Example ItemID: 228853
