Remove or coment the lines 512, 513, 514
Save file.

this seems to be an issue with the logo image, line 513, edit the image source to docs/images/pcbgcode-600.gif width='300' and line 570 edit the image source to docs/images/isolation_settings.gif width='300' and finally edit line 661 the image source to docs/images/z_axis.gif width='300'

you only need to remove the single quote around the img src name not width

With what Eagle version did you run into this issue?
I can only get pcb-gcode to work with Eagle < 8.x
