Entering the blue womb from the utero entrance after It Lives has the trapdoor turn into the red womb trapdoor during the descent animation.
During the transition cutscene, the chest spawn sfx occurs immediately.
The three question marks (???) used to convey that your now on the stage do not occur upon the floors start.
The blue womb stage graphics (such as being slightly darker, and the hush branch/vein things that occur when he is low health) do not occur during the floor.
These issues are not present when entered from womb or scarred womb, only utero.
This has been tested with stage graphic mods, which amplifies these issues, and also with only stage API, where the issues continue to remain even there. So this is a StageAPI specific bug.
