The first version's hierarchy different from the HEAD's #2149
Description
Why does the newly created version (the 1st one) of my demo source have a hierarchy different from the HEAD version’s although nothing is changed to the content?

In other words, why would the hierarchy get messed up?
Expected Behavior
Comments by Joseph Amlung The hierarchy should be appearing the same in both HEAD and in the version.
Current Behavior
The hierarchy would showed diferently in the HEAD version and the 1st version.
Comments by Joseph Amlung The hierarchy_root_url and hierarchy_meaning looks correct in both the HEAD and in the source version, and the concepts are both traversing down the hierarchy correctly.
Possible Solution
