OnFireOutput hook ( name, ent, activator, param, delay ) - fired when an map IO output is fired.

Yeah, I was afraid of that, thanks for testing. I guess we won't be having this feature after all. I am not modifying every single entity in the whole engine to store extra data on all the outputs.
Remove OnFireOutput hook, it will never work 100% correctly
But it was very cool to see all the outputs, even those that don't have links to input. I want to create a binary module for fun. Maybe there will be time for this
