Updated API & fixed examples

#6 - Fixed examples and updated API spec

• re-generated code using swagger-codegen 2.2.3 so as to fix the generated examples
• minor documentation changes for API v1
• bunch of documentation changes/updates for API v2
• enforced restrictions on associations endpoints for users, systems, user groups and system groups
• minor non-breaking changes in the generated code due to the swagger-codegen upgrade
