Inline Bibliography form doesn't use rich text editor for title if returned with errors

Steps to reproduce:
* On `/bibliography/` click 'Create new'
* Enter a title and nothing else, then click 'Create'
* Form gets returned because of missing data, but this time without the rich text editor for the Title field
* Also, `Create new` button reappears prematurely
