Merge WISCENTD app with NHWA app (import excel templates of HWF)

Update excel template for Module 1 (NHWA has created a new dataSet with different dataElements)
Second release
- Add new button to allow downloading the excel template filled in with data (querying the dataValueSets API)
Third release
- Complete dataSets upon importing
- Option 1: Always completing with current date
- Option 2: Completing dataSet with current date in the web app (checkbox)
- Option 3: Allowing adding a custom completion date in the excel template
- This requires modifying all the templates!
