ux: prerequisites not surfaced before Quick Start — Node version and API key silent failures

Problem

New users hit the Quick Start before seeing what's required. Two silent failure modes:

1. Node version badge visible but no prereq section. User on Node 16 installs, fails with cryptic error.
2. ANTHROPIC_API_KEY not mentioned in Quick Start. squads run silently fails or shows unhelpful error if key is missing.

Proposed Fix

Add before Quick Start:

Prerequisites: Node.js >= 18, ANTHROPIC_API_KEY set in environment, a git repository.
