[Bug]: Can't use, getting forge ZSH Segmentation Fault

Bug Description

Tried installing forge for the first time, am getting ZSH segmentation fault after choosing provider (tried both github copilot and vertex AI). forge zsh doctor runs fine and install also runs fine. Any clue? Installed forge following website reccomendation (curl -fsSL https://forgecode.dev/cli | sh), also noticed this github issues modal shows npm options?

Happens after choosing modal provider during initial setup following docs getting started section (https://forgecode.dev/docs/)

Regular Mac Terminal Zsh

Same thing happens on vscode zsh.
I am trying this harness on my work machine, I will test later on my personal mac. Could this be a company computer software issue? A colleague tried in his and had the same segfault (same company, similar macbook)

Also could not find a way to easily install an older version to debug version issues

(Still happens on 2.4.0)
