Allow Slate forms to be embedded

Motivation

The University's webform solutions include Slate in addition to Trellis. It should be easy to utilize Slate forms on a website.

Proposed Resolution

Describe the solution you'd like

Create the ability to add a media item with a Slate form in it. Utilize the same functionality as the Trellis forms media type.
