qr code invalid when setting up TOPT

We are trying to set up TOPT app authorization. I have the following active.
Authenticator app |   |   | 100 | Settings | Yes | Yes
E-Mail Factor |   |   | 100 | Settings | No | Yes
Grace period |   |   | 100 | Settings | No | No

The email factor is working, but when I try to scan the QR code on a user, it comes back with an invalid QR Code. I've tried the Microsoft and Google Authenticator.

I'm missing something?

This is Moole 4.1.5 latest MFA plugin.

AH, I see I'm trying to scan the moodle app qr. I should be seeing setup mfa under preferences, that has its own qr code. Its not showing up when I log on as a test user.
