Vendor ID # (This should be auto-populated by the system at some point, either upon starting the form or possibly better upon the vendor completing and submitting the form).

Shipping to Country (a scrolling list of all countries including choices of “All” , “United States only” , “United States & Canada only”) They can pick as many as they want and those countries populate in a window that allows them to choose “Free Ground Shipping” checkbox on a per country basis to the right of each country.
And, somehow, we have to allow the United States entry to open up and allow for different treatment of Alaska/Hawaii. I think if the vendor says free shipping to U.S. in change style to fit crowdid #10 above, it just needs to allow them to uncheck this treatment for these two more expensive destinations.
