Wire setup-pm to Azure DevOps (KB guidelines shipped in #235, config flow not wired)

Problem

Story #235 (PR #317) shipped a full Azure DevOps guideline set (4-axis: implementation, issue-management, automation, project-tracking + a canonical-states mapping example) — but pair-capability-setup-pm still asserts Azure DevOps has "No implementation guide yet" and lists only GitHub Projects + Filesystem as supported tools. A project running the interactive setup flow cannot select Azure DevOps even though the KB now fully documents it — discoverability today only comes from reading the category READMEs directly.

Deliberately deferred out of #235's scope (mutex with parallel skill-layer work at the time).

Fix

Wire Azure DevOps into packages/knowledge-hub/dataset/.skills/capability/setup-pm/SKILL.md (+ installed mirror .claude/skills/pair-capability-setup-pm/SKILL.md):

• Add Azure DevOps to the supported-tools list.
• Point to the 4 guidelines shipped in Azure DevOps as supported PM tool (4-axis guidelines + state-mapping example) #235 (project-management-tool/azure-devops-implementation.md etc.) for the implementation-guide reference the skill currently says doesn't exist.

Small, mechanical — should not need a full story breakdown.
