[BUG] Image Output Filename Format

Oh, i didn't realized, but this is another bug.

The upscaled image's name isupscale_1742486913_42_512x512.png which is wrong... The resolution in the filename is very wrong.

/xxx/xxx/xxx/sd_ui_output/upscale/upscale_1742486913_42_512x512.png

Format in settings:
[mode]/[mode]_[jobid]_[seed]_[width]x[height]

Source image resolution is 768x512
Upscaled image resoultion is: 3072x2048

I reckon the offending code originates here. It doesn't take into account the item.mode, and therefore doesn't recalculate height and width using the item.params.upscale_factor when the mode is "upscale".
