on this scenario, it only adds one record...

"us","FL","Salem"
"us","GA","Salem"

Same city name but different state. How can this be fixed?

Hi

The module as-is does not support duplicate terms with the same name.

If you want that you'd have to modify the duplicate check to not just check for an existing term but an existing term with the same parent: https://github.com/grahl/term_csv_tree_import/blob/master/src/Controller/ImportController.php#L24
