h264 Decodes Failing

Transcodes requiring the h264_cuvid decoder are failing with the following:

[h264_cuvid @ 0x7f9f65f56840] ctx->cvdl->cuvidParseVideoData(ctx->cuparser, &cupkt) failed -> CUDA_ERROR_UNKNOWN: unknown error
ERROR: decoder.c:64] Error sending packet to decoder : Generic error in an external library
ERROR: transcoder.c:207] Could not decode; stopping : Generic error in an external library
E0605 17:03:47.585189       1 ffmpeg.go:374] Transcoder Return : Generic error in an external library
E0605 17:03:47.585303       1 ot_rpc.go:188] Unable to transcode Generic error in an external library

This is occurning on:

• Windows 10 GTX 1070 Ti running unpatched and patched 466.63 driver with 0.5.14 and 0.5.18 precompiled binaries
• Ubuntu 20.04.2 LTS (GNU/Linux 5.8.0-55-generic x86_64) Quadro P400 running unpatched and patched 460.80 driver with 0.5.14 and 0.5.18 precompiled binaries and docker images

I also have been getting this sporadically. It looks like my machine has two versions of cuda installed, which may be the issue. Can either of you confirm you're in the same situation?

A reboot resolved the issue temporarily but I'm not sure what causes it. I'll be transcoding just fine and the issue occurs.

Nvidia Driver: 495.44
CUDA Version: 11.5
