Cleanup: remove run_combo tool and wire bash-based execution

Remove the `run_combo` tool and update tool registration/agent behavior to use the bash tool with `coco combo run`.

Acceptance:
- `run_combo` tool is removed from tool registry and executor special handling.
- Tooling/docs updated to point to `coco combo run`.
- No regressions for combo execution paths.
