Add real QVAC validation checklist

Production-readiness goal: define the manual validation needed before calling qvac-bench production-ready.

Acceptance criteria:

• Add docs/validation-checklist.md covering setup, QVAC server URL, model selection, expected command, expected output, and failure triage.
• Include a matrix for at least one Mac local run and one fresh install run.
• Clearly separate mock CI validation from real QVAC validation.
• Do not require CI to have a real QVAC server.
• Mark any unresolved manual checks clearly.

Safety notes:

• Do not auto-merge if the issue expands into claims about real performance without actual run data.
