docs(AGENTS): add Layout conventions section#9

Specializes the org-level layout rule for m-standard's top-level layout. States the cross-repo rule (docs/ is prose-only) and maps each top-level dir to its purpose: integrated/ for reconciled output, per-source/ for provenance, schemas/ for validation, sources/ for upstream replicas, mappings/ for cross-source name maps, dist/ for the Phase 0 contract. Links back to .github/CONTRIBUTING.md § Layout conventions for the org-level rule.
