docs: PyPI version refs 0.19.4 -> 0.19.5 (post-republish)

The 0.19.5 PyPI republish landed after the earlier version-propagation commit,
so the "current PyPI version" refs in CLAUDE.md + llms-full.txt were left at
0.19.4. Bump to 0.19.5. npm 0.20.0 / NuGet 0.19.3 / Maven 7.11.3 unchanged.
