Create trial design domains

Create and export the trial design domains for the SoA. Now these domains can be created:

TV
TA
TE

Start with these and ensure they are SDTM-compliant

The corresponding mapping from USDM to all these domains is stored in Excel as stdm_mapping.xlsx and can be found in the DDF-RA GitHub Documents/Mappings directory of the corresponding USDM version

Implemented the creation of TA, TE and TV. This data is stored for each study designed in the SOA. TI, TS (and others) cannot be created at the moment because the relevant data is not captured in the SOA Workbench.

Unit tests have also been added as well as the UI pages.
