Add custom provider for nest integration

Currently scout_apm_node has ALPHA level support for nest -- it would be nice to offer some deeper integration by providing:

• An app scope provider that makes the Scout instance available
• Request scoped provider that makes the ScoutRequest and/or ScoutSpans accessible.
