Add per-cavity note PV to screen with fault details 

Please add a way to display the content of the new per-cavity note PV (e.g. ACCL:L3B:3580:AUTO:NOTE) to the screen with fault details that opens when clicking on a given cavity. Possible locations either at the top or bottom of the window(?)
