Add request / response / error logging

• add run time server side request / response logging (stdout is fine)
• add persistent error logging / tracing
• add monitoring

there's a good start on a basic request logger now, just want to dial in the formatting a bit. It's currently a bit verbose for debugging purposes. Haven't started on any persistent logging setup.
