Feature/icon sizing issue fix

Description

Replaced all instances of the non-standard w-4.5/h-4.5 Tailwind class across the codebase with standard w-4/h-4 (icon-md, 16px). The 4.5 size is not a standard Tailwind token and may render inconsistently. Found and fixed 9 occurrences across 5 files: MerchantSidebar.tsx (logo + nav icons), Topbar.tsx (notification bell + theme toggles), and 3 loading skeleton files.
