Add comprehensive tests for: - Exception chain flattening (25 tests): parsing, classification, actionable hints, and edge cases - Adapter configurations (40 tests): Node.js, Python, and .NET adapters including launch/attach configs and exception filters - Fix type errors in existing manager.test.ts mock adapter Total tests increased from 87 to 152.
