docs: strengthen offline agent report contract

Summary

• Expands the Implement offline-agent policy UI and return report #149 offline-agent policy and return report source doc with player-facing policy surface, effective-policy handling, return report truth-source rules, stale policy guard, UI placement, evidence, and cut line.
• Strengthens the offline-agent evidence template for requested/effective policy, server-owned validation, row mapping, policy/report correlation, and risk honesty.

APPROVED, docs-only. Scope is limited to #149 offline-agent policy/report design and evidence templates. No Unity UI, Nakama runtime, backend policy logic, gameplay authority, reward, inventory, TIME, SECOND, body, quest, relationship, memory, or LLM behavior changed. The update strengthens the player-facing contract and makes clear that report rows require server activity, ledger, policy, Garden state, fallback, or explicit simulation truth sources.

Closes no issue. Issue #149 remains open until Unity UI and Play Mode evidence exist.
