docs: lock gate outcome row mapping contract

Summary

• Expands the Add Gate outcome report ledgers for living roster consequence #137 Gate outcome report design with a row-state contract for server-backed, explicit-zero, denied, debug-simulated, not-implemented, and hidden-by-cut rows.
• Defines required server sources for result, entrants, SECOND, TIME, body condition, proof, temporary loot, memory, relationship, Garden consequence, recommendation, and ledger refs.
• Strengthens the gate-outcome-report evidence template so testers can prove row mapping, authority boundaries, reward/TIME truth, and known gaps.

APPROVED, docs-only. Scope is limited to #137 outcome report design and evidence templates. No Unity UI, Nakama runtime, reward logic, TIME/SECOND mutation, body lifecycle code, memory, relationship, inventory, quest, Garden state, or LLM behavior changed. The update reduces implementation ambiguity by requiring every visible outcome report row to map to server-owned state, explicit zero, denial, debug simulation, not-implemented label, or hidden-by-cut evidence.

Closes no issue. Issue #137 remains open until runtime outcome rows, Unity UI evidence, Nakama ledger validation, and Play Mode evidence exist.
