Align manual verification with the current title section

Summary

• align the MV3 Markdown contract with the current Source provenance fields
• verify page titles in the separate Untrusted Page Title section and its dynamic inert fence
• remove obsolete title-under-Source requirements from the guide and result template

Scope

This PR addresses GPTPro re-audit finding F006 only. F003, F004, runtime extraction, Markdown formatting, permissions, dependencies, and workflows are unchanged.

• bundled Node --test — 40/40 passed
• repository sanity static checks — passed
• git diff --check — passed
• generated-Markdown truth probe — all seven Source fields present, title separate, dynamic fence confirmed
• repository search — obsolete title-under-Source wording 0
• security triage — docs-only three-file diff; no runtime, manifest, workflow, package, permission, or communication-path changes

Boundaries

• no F003 or F004 changes
• no runtime extraction or permission changes
• Draft and unmerged pending maintainer review
