The Adaptive Release!

The HPS method implemented here is now fully adaptive. Given a p4est tree with ForestClaw patches, this will solve the elliptic problem given (Poisson's equation) on the fully adaptive tree.

Coarse-fine interfaces are handled via a "coarsen-patch" approach.
