Summary

Closes #8686

Scope

  • The PR title follows type(scope): short summary Conventional Commit format, for example fix(api): restore profile access checks.
  • This PR is focused and does not mix unrelated backend, UI, MCP, docs, dependency, and deploy changes.
  • This follows CONTRIBUTING.md and does not reintroduce GitHub Pages, VitePress, site/, or CNAME.
  • I linked a currently open issue this PR resolves (e.g. Closes #123) - a linked open issue is required for every contributor PR.

Validation

  • git diff --check
  • npm run actionlint
  • npm run typecheck
  • npm run test:coverage locally; codecov/patch requires ?99% coverage of the lines AND branches you changed (aim for 100% on your diff so CI variance does not fail near the threshold). Global coverage is a non-blocking trend with a loose 90% backstop, not the gate.
  • npm run test:workers
  • npm run build:mcp
  • npm run test:mcp-pack
  • npm run ui:openapi:check
  • npm run ui:lint
  • npm run ui:typecheck
  • npm run ui:build
  • npm audit --audit-level=moderate
  • New or changed behavior has unit/integration tests for new branches, fallback paths, and sanitizer boundaries

If any required check was skipped, explain why:

Safety

  • No secrets, wallet details, hotkeys, coldkeys, user PATs, private keys, raw trust scores, private rankings, or private maintainer evidence are exposed.
  • Public GitHub text stays sanitized, low-noise, and does not imply compensation guarantees or optimization tactics.
  • Auth, cookie, CORS, GitHub App, Cloudflare, or session changes include negative-path tests.
  • API/OpenAPI/MCP behavior is updated and tested where needed.
  • UI changes use live API data or real empty/error/loading states, not production mock/demo fallbacks.
  • Visible UI changes include a UI Evidence section below with JPG/JPEG or PNG screenshots arranged as organized, captioned, clickable thumbnails. SVG screenshots are not used as review evidence. Review-only screenshots or recordings are not committed to the repository.
  • Public docs/changelogs are updated where needed; changelogs are only edited for release-prep PRs.

UI Evidence

N/A - no visible UI changes.

Notes

When both the primary gate check publish and its errored fallback fail, push gate_check_run onto failedOutputs so gate-only repos get the same pr_public_surface_failed / PostHog / transient-retry path as check_run.

Review summary
This PR closes #8686 by making the gate_check_run publish path push a failedOutputs entry when both the primary publish and its errored-fallback fail, mirroring the existing check_run/comment/label contract so gate-only repos correctly escalate to pr_public_surface_failed/PostHog/transient retry instead of silently leaving failedOutputs empty. The two new `!gateFinalized` guards are placed at the two call sites where gateFinalized would otherwise remain false after a double failure, and the diff updates/adds tests that exercise the real double-403 and double-500 paths (not fabricated states) plus asserts on audit metadata and the PostHog capture call. This is a narrowly-scoped, well-targeted bugfix with matching test coverage for both the permission_missing (non-transient) and API-failure (transient) branches.

Nits — 4 non-blocking
  • The `fix(orb): gate_check_run publish failures never populate failedOutputs, unlike check_run/comment/label #8686` inline comments in src/queue/processors.ts:10813 and :10863 are fairly long block comments explaining historical context — consider trimming to the non-obvious 'why' only, per repo convention of terse comments.
  • The first modified test (queue-4.test.ts) drops the previous audit-write-failure coverage entirely rather than adding the new scenario alongside it — worth double-checking that behavior (audit write failure during incomplete-surface) is still covered elsewhere.
  • Consider factoring the repeated `failedOutputs.push({ output: "gate_check_run", ... })` block (appearing twice, at 10813 and 10863) into a small local helper to avoid drift between the two call sites, similar to how check_run/comment/label presumably share one shape.
  • Double-check that `gateFinalized` can never be true after an exception path reaches the second block — if it can, the added guard is dead code.

Addressed
The diff adds failedOutputs.push({output: "gate_check_run", ...}) in both the primary-catch and no-fallback branches of maybePublishPrPublicSurface, matching the check_run/comment/label contract, and adds/updates queue tests covering permission_missing and transient double-failure paths that assert pr_public_surface_failed, PostHog escalation, and retryable transient errors fire.
