fix: keep bare Codex PermissionRequest events in working status - #4

Codex emits PermissionRequest even when an existing policy is about to auto-approve the tool call, so a bare event only proves the agent is still processing — it no longer flips the session to waiting. Explicit requires_action / waiting_for signals in the payload still map to waiting as before, and Claude Code behaviour is unchanged.
