Advisory-vs-block classification of health-gate violations now rides typed entry metadata instead of substring-matching the operator message — a message reformat can no longer silently flip enforcement. health_gate{gate,tool,enforcement}metadata and classification reads ONLY that (engine.py) — a message reformat can no longer silently flip advisory↔block.
