Summary
Adds CLI commands for fetching Outcome Pack setup docs, listing/searching available packs, and fetching a pack by exact name. The command docs share the existing markdown fetch behavior with a new outcome docs base URL override, and the outcome setup skill now resolves packs before scaffolding files. Tests cover default and overridden URLs, selector validation, and fetch failure messaging.
