当前 README 中的 Star History 图表因 GitHub stargazer API 限制已无法正常展示，影响项目星标数据的可见性。 本改动将 README.md 和 README_EN.md 中的图表地址及跳转链接更新为可正常工作的新服务地址，并保留原有参数，确保图表重新可用。
