Upgrading from a pre-UUID server currently fails with a bunch of web request errors because the API endpoint for converting player names to UUIDs has changed. I've updated it to the latest version, as seen here: http://wiki.vg/Mojang_API#Playernames_-.3E_UUIDs

I also did a bit of cleaning up to avoid querying the same player name multiple times, and moved the logic for request batching and rate limiting into
UUIDFetcher.
