Phase 5 of unified org settings: lean tier repos require check names `build` and `codeql` to match the org ruleset contract. Adds CodeQL (javascript-typescript for docs JS), OpenSSF Scorecard via centralized caller, and top-level permissions/concurrency to lean_action_ci.yml.
