Fix seen-line frontier tracking

Summary

• mark seen-line frontier consistently on completion and on system-driven advance
• treat the frontier line itself as seen and document the section-level frontier model
• add regression coverage for selector and action flows, and keep line-entry rendering correct for presentation-only actions
