Fix PR action loading states in review sidebars#170

- Track the active PR write action so only the matching button shows a spinner.
- Thread that state through the shared git-review hook and both review sidebars so merge, close, ready, and update actions stay distinct.
- Keep the other PR controls disabled while a request is in flight to prevent overlapping writes and confusing feedback.
- show the spinner only on the PR write action that is running - thread pending action state through the Git and PR review sidebars
