fix(node): enforce hard timeout after continue

• enforce hard-timeout handling after timeout-continue and force peer timeouts once
• extend RequestTag hard-timeout coverage for opennet waits and orphaned senders
• add tests for hard-timeout timing and sender-loss fallback
