docs: 说明文件化 Plan 协作流程

摘要

本 PR 同步中英文用户文档，说明 Plan 是工作目录中可审阅、可修订、可版本化的文件，而不是需要进入/退出的模式或整份方案审批流程；同时说明团队目录的真实激活、可见标识、成员继承和高可靠 team_only 模式。

具体变更

• 重写计划协作章节，说明 .desirecore/plans//PLAN.md、revisions、自然语言修订和安全边界。
• 明确“认可 Plan”不等于执行授权；发送、发布、删除等动作仍在实际发生前走审批 / Human Gate。
• 修正委派模型、教学六原语与执行监控中的旧语义：没有统一“批准计划”状态；用户可说“先只写计划”；Task 只在责任、阻塞、跨 Agent 协作或持续进度确有需要时创建，不机械复制每个 Plan 步骤。
• 说明普通聊天点击团队目录会为当前具体 conversation 激活 teamId，并在输入框胶囊与右上角资源入口显示团队标识。
• 说明每个会话的团队选择彼此隔离。
• 说明团队创建自动建立共享目录绑定，组长、初始成员、后续成员和新主管动态继承，非成员不可见；绑定失败则创建失败。
• 说明“仅使用团队工作目录”：团队目录成为唯一项目工作目录和 Plan 根，成员/全局目录不进入 related_workdirs，但原设置不会被删除。
• 中文原文与英文 i18n 镜像同步更新。

验证

• npm run build 通过；Docusaurus zh-Hans 与 en 均成功生成。
• git diff --check 通过。
• 旧的 blanket plan approval / every Plan step becomes Task 表述已全仓检索清理。
• 当前 head：9cfd3b4c94a3cb62631835ee451cd681d720ab6b。

• Reframes Plan as a versioned, user-editable workspace artifact rather than a mode or blanket approval state.
• Separates natural plan review from action-level permissions and removes the rule that every Plan step becomes a Task.
• Documents visible, conversation-scoped team activation and dynamic membership inheritance.
• Documents the non-destructive team-only workspace option in both Chinese and English.
