docs(skills): restructure spec-start qna.md as flat Q-NN log

Summary

• Replace the old three-part qna.md layout (Assumptions / Open questions / Scope) with a flat stream of ## Q-NN entries and Kind: bullets (assumption | open | scope | decomposition).
• Answers are optional sibling ### headings under a question; exactly one example ### is seeded only under Q-01 so the file stays easy to scan before review.
• spec.md now calls for a Q&A index pointing at Q-NN anchors; checklist, process diagram, self-review, and aftercare are aligned.

Setting a structure for the qna.md file that can facilitate including answers is necessary for implementing #802
