docs: add D-ID avatar plugin to plugin lists

Description

Added the new D-ID avatar plugin (@livekit/agents-plugin-did) to the plugin lists in CLAUDE.md and README.md, which were missing from the documentation after the plugin was added.

• Added 'did' to the Avatar plugins list in CLAUDE.md
• Added @livekit/agents-plugin-did entry to the plugins table in README.md
